How do I Request a Self-Study Activity?

After browsing our list of educational activities and selecting your choice, your first step is to complete the Continuing Education Standardized Request Form.

It is NECESSARY to complete this form in order to receive access to the educational activity you have chosen.

Completion of the Continuing Education Standardized Request Form Purpose:

To complete a needs assessment as to why the educational activity is being requested.

Guidelines:

1. The price of each activity is $35.00 USD. There is no charge for Chartwell associates.

2. Complete a Continuing Education Standardized Request Form for each activity you want to purchase. E-mail all of them to Kathy Hammond as directed on the form.

3. Once approved, you will receive an e-mail confirming approval. Then you need to click on the "PayPal ADD TO CART" button on the applicable activity's web page to order and pay for the selected activity/s. You can order just one activity or multiple activities at the same time.

4. Once we receive confirmation that your payment has been processed you will be e-mailed the applicable passwords for the activities you ordered.
