Ailing individuals need to focus their energies on healing, not on unanswered questions, paperwork or other outside distractions. What makes Chartwell unique among other home healthcare providers is how we represent this philosophy in our model for care delivery, which is based on two of our core values: teamwork and communication.

Once home care is prescribed, our infusion/respiratory branches assign a Chartwell Care Team to design and monitor each patient's care plan. Heading this team is a clinician, usually an R.N. or respiratory therapist, who acts as primary caregiver and liaison to the rest of the Care Team. With this multi-disciplinary approach, we’re able to notify the physician of a dietary deficiency, identify drug interactions, or communicate with a payer on behalf of a patient.

The Chartwell Care Team seeks and embraces every opportunity to communicate with others concerned with the patient's welfare, including family members, physicians, hospital nurses, case managers, payers or government health agencies. Open communication with these groups is the key to providing excellence of care and achieving better clinical outcomes.
